Students who are licensed drivers may only drive themselves to-and-from events at HMS with written permission from their parent/legal guardian (By selecting "yes" above, you acknowledge that this form serves as your written permission. You also agree to the Student Driver Policy outlined in the following text). They must sign themselves in and out with History Museum and Discovery Center staff members. If leaving early, HMS management must receive notification and permission from a parent/guardian. Student drivers are advised to park in the public all-day parking garages near the museum, located on College street.

MBU Policy

You will receive an additional form in a follow-up email regarding event permissions, hold harmless agreement, and photo release. Please read, sign, and submit the completed paperwork on or before the day of the event. Both this digital information form and the signed paper form must be completed and submitted to the History Museum on the Square for your scout to participate in the MBU event.
